ALERT:  TTo meet the Oregon Governor’s requirements, the Douglas County Clerk will be limiting walk-in traffic, requesting customers wear face covering and continuing social distancing when visiting our office.  During the COVID-19 crisis, services may also be offered through phone, website, email, mail, and curbside (by request).  If services can’t be accommodated by the aforementioned methods, they can be by appointments or walk-in.

NOTE:  Effective January 1, 2018 the fee for each copy of a Birth Certificate or Death Certificate is $25.00 per each copy regardless of the number of copies ordered at one time.  Please click here for 2017 and new (eff. 01/01/2018) fees!


Certified copies of Birth and Death certificates are only available from the Douglas County Clerk’s Office from the date of the birth or death until the day before the event turns six (6) months. (When the date from the birth or death reaches six (6) months, we have no access into the State’s system to generate the birth or death certificate.) If ordering a birth or death certificate within the first six months from the date of the birth or death, please download a Douglas County application/order form from one of the following links:

OR, you can pick up an application by visiting Douglas County Vital Records, Monday-Friday, 9:00am to Noon and 1:00pm to 4:00pm at the following location:

A completed order form and full payment must be received at time of order.  Certified copies are, whenever possible, issued within one working day following receipt of a request. To ensure confidentiality, all applicants are required to show picture identification.  Personal checks or money orders are acceptable if made payable to: Douglas County Clerk. Visa/MC and Debit cards are also accepted.  We will not accept applications without full payment.
