THE POSITION: Provide case
management for individuals with mental illness including those
dually diagnosed with substance use issues and/or developmental
disabilities. Case management and other treatment services are
provided in concert with individual’s MH primary clinician,
other treatment providers and community partners. Essential
Functions: Provide support in achievement of goals and
meeting basic needs; problem solving, skill building and
development; serve as an advocate, assist individuals in
obtaining necessary services and benefits with community
resources and partners; on-going communication for provision of
treatment; documentation of interventions, progress, and
observations; under direction of MH primary clinician,
implement, coordinate and monitor progress towards goals in
individualized service and support plan; facilitate group
interventions; education and dissemination of information to,
and for the benefit of, individuals with mental illness;
organize, gather, maintain and complete documentation, reports
and statistical information as identified and required by
department policy and state law; network with professionals in
other programs and agencies as well as referring physicians and
programs.
Must adhere to a professional code of ethics such as those
promulgated by the National Association of Social Workers.
Requires regular and consistent attendance.
STARTING SALARY RANGE:
$2,534 - $2,906 per month plus excellent benefit package,
including retirement.
MINIMUM QUALIFICATIONS:
Bachelor’s degree in behavioral science (psychology or social
work) or related social science plus two years experience in a
mental health setting or a satisfactory equivalent combination
of education, experience and training. At time of appointment
must posses a valid Oregon Driver’s License.
In addition
to the above requirements, applications will receive further
evaluation based on thorough knowledge of: social
service theory, practices and procedures to be able to identify
a problem, decide on an appropriate action, and provide
indicated services; considerable knowledge of human behavior as
it relates to crisis situations and problems of client
population; reasonable knowledge of laws relating to mental
health; considerable knowledge of sound case management
principles, practices and procedures; considerable knowledge of
behavior intervention, management and support strategies;
considerable knowledge of community resources, functions and
procedures of other social service agencies and service
providers; reasonable knowledge of educational theories and
techniques. Skill in: Writing clear and concise
correspondence, records, treatment plans, reports and
statistical data; interviewing, counseling and listening
techniques and principles; and family and group dynamics.
Ability to: Communicate effectively both in written
and oral form; plan/consult/coordinate with physicians, source
agencies, families of clients and the community; participate in
diagnostic and treatment conferences within an
inter-disciplinary team; work with and relate to a diverse
population of people with mental and physical disabilities; work
independently, efficiently organize time and meet established
deadlines; remain calm and use good judgment during
confrontational or high pressure situations; make decisions
independently in accordance with established policies and
procedures and use initiative and judgment in completing tasks
and responsibilities; utilize problem identification and
resolution techniques; teach socialization and living skills to
individuals with various learning levels; establish and maintain
records, reports, and statistical data; courteously meet and
deal effectively with other employees, clients, families,
schools, service agencies, service providers, physicians,
professionals, team and committee members and the public.
PHYSICAL DEMANDS AND WORK ENVIRONMENT:
Work is performed in an office environment and in the field; may
involve hearing voice conversation, color identification,
keyboarding, gripping with hands and fingers, lifting up to 14
pounds, bending, reaching, sitting, standing, walking, and
operating a passenger vehicle.
PHYSICAL EXAMINATION:
Offer of employment is contingent upon successful completion of
a pre-employment chemical detection screen conducted at County’s
expense.
CRIMINAL BACKGROUND CHECK:
The finalist for this position will be made an offer of
employment, contingent upon passing a full criminal history
record check in accordance with OAR Chapter 407, Division 7. A
disqualifying event determined by the State of Oregon, will
result in denial or termination of employment.
DRIVING RECORD:
Applicants selected for interview will be required to bring to
the interview a recent Court Print driving history obtained from
the DMV. Please allow 5-7 days for delivery and longer for out
