DOUGLAS COUNTY INSURANCE ELIGIBILITY/OPTIONS
January 1, 2008 through December 31, 2008

This will provide you with basic information concerning your insurance benefits.  Your insurance is effective three months after your date of hire/promotion into a benefited position.  If your hire date falls on the first through the seventh day of any month, the entire month is counted as the first month of your three-month qualification period.  For example, if your hire date is between January 1 and January 7, your insurance would begin April 1.  If you begin work after January 7, your insurance would begin May 1.

Medical/Dental Plans:  Full-time employees are eligible for the current County contribution of $999.41 per month toward the cost of full-family medical and dental plan coverage.  There are two medical options from which to choose:  a managed care plan (Prime High) and a new Health Reimbursement Account (HRA).  Our medical insurance carrier is PacificSource.  Dental options include the choice of either a traditional dental plan (Oregon Dental Service) or a managed care dental plan (Willamette Dental Insurance).   This results in six potential combinations from which to choose.  Your payroll deduction amount will depend upon which combination of plan options you elect; see the attached rate table.   To assist you in deciding which plan combination is best for you and your family, we provide a table comparing benefit descriptions of the various options within this employee packet.

Employees in positions that are budgeted to work at least 20 hours per week, but less than 40 hours per week, are eligible to receive contributions by the County on a pro-rated basis (50%-90%).  The County contribution correlates to the budgeted FTE, (i.e., 20 hr/wk = .50 FTE; therefore, the County would contribute 50% of the amount allowed a full-time FTE, if the employee agrees to pick up the remaining amount through a payroll deduction).

Pro-rated employees may waive medical/dental insurance.  If you do, you will be asked to sign a waiver.  If, at a later date, you wish to enroll in the insurance plans, you may do so at open enrollment each year.  Open enrollment occurs in November/December of each year, with coverage effective January 1.  You may apply during the year if your application is due to the involuntary loss of other health coverage or acquire new dependents, as long as you make application and coverage with the County plan begins within 30 days of the date of the event.  If you do not apply for coverage under the County plan within 30 days following the termination of your prior coverage, you may enroll for coverage during the open enrollment period referenced above.

Life Insurance:  Life insurance through Standard Life is provided to benefited employees at no charge.  The employee is covered for $10,000 (plus additional for accident), and $2,000 for dependents age six months and older.

Select Your Benefit Package:  Complete the “Health Plan Selection and Authorization” sheet and accompanying enrollment forms; submit them to Human Resources within the first 30 days of employment

Voluntary options available to employees include:

Flex Spending Account:  Employees have the option to participate in a Flexible Spending Account (Section 125) administered by Manley which offers the ability to use pre-tax dollars to pay for eligible health and dependent care related expenses.  More information on this is found elsewhere in this packet.  If you choose not to participate at this time, please complete and return the enclosed waiver.  This does not preclude your participation in future calendar years.

·                     Cancer/ICU Insurance (AFLAC)

·                     Deferred Compensation  (available companies: ING, Nationwide, Oregon Savings Growth)
