Douglas County registers all
Births and Deaths that occur in Douglas County. Certified
copies of Birth and Death certificates are available for up
to six (6) months after the date of the event at the Douglas
County Vital Records Department in Roseburg. You can
download an application/order form to complete by
clicking the appropriate DOUGLAS COUNTY link below

Certificates may be
purchased for $20.00 for the first copy, plus $15.00 for
each additional copy ordered at the same time. A
completed order form and full payment must be received at
time of order. Orders and payments received before
4:30pm will be processed that same day. To ensure
confidentiality, all applicants will be required to show
picture identification. Personal checks or money orders are
acceptable if made payable to: Douglas County Vital Records.
Visa/MC and Debit cards are also accepted. We will not
accept applications without full payment.
