Our Goal
The goal of the Maternity Case
Management program is to lower risks for the woman and her
baby, and make sure she gets prenatal care by a health care
provider.
What We Do
When
a woman sees a Maternity Case Management Home Visit Nurse,
she is provided personalized help to ensure a healthy
pregnancy and delivery. Home Visit Nurses work with
women to assess and manage their health, social, economic,
and dietary concerns. With the help of the a Home
Visit Nurse, pregnant women learn to identify and develop
the strengths, resources, and skills they need to improve
their own health and the health of their baby.

Health Nurse during pregnancy and after the baby is born.

tips to help keep children safe

Diet and nutrition
education and assessment

Assessment of emotional
needs, relationship support, counseling, and referral as
needed.

Motivational approach to
helping women identify risks and solutions to ensure a
health pregnancy and birth.

goal-setting to promote healthy pregnancies and positive
lifestyle choices.
