The Douglas County CaCoon Program is a care
coordination program provided by Public Health Nurses with
specialized training. The CaCoon Program is a program for families that have children with special health needs. CaCoon stands for
Care Coordination to help parents find resources and to coordinate services.
CaCoon serves families with children and young adults (birth to age 21) who have complex health conditions, such as cleft lip/palate, spinal bifida, cerebral palsy, heart disease, rheumatoid arthritis, chronic orthopedic problems, and other special health needs.
The goal of the CaCoon Program is to help
families learn new skills to become as independent as
possible in caring for their child with special health care
needs. A trained CaCoon Nurse coordinator can help families with:
- Screening and monitor children who are at risk

questions about a child's health needs and special care

Identify developmental problems early

Find important
special services close to home

Teach special techniques for caring for your child

Work with
your child's primary care provider to coordinate health care
and specialty services

support in times of stress and crisis
