MINUTES of the board meeting of September 15, 2004 to be approved.

CITIZEN PARTICIPATION - Citizens wishing to address the commissioners concerning this agenda, or items of interest not included on this agenda, may do so at this time. When recognized, anyone addressing the commissioners will go to the microphone and give your name and address for the record.

PROCLAMATION establishing Family Day in Douglas County, on the fourth Monday of every September, to be held this year on September 27, 2004.

HEALTH AND SOCIAL SERVICES

1. Agreement with the North Douglas School District for Douglas County Health and Social Services to provide mental health treatment in the school.

HUMAN RESOURCES

1. Modification of contract with Mercy Medical Center for employee assistance services to provide counseling and work life education to Douglas County employees. Contract is for the period of October 1, 2004 through September 30, 2005. Payment to contractor not to exceed $16,180.

LAND

1. Order to accept deed from the United States Department of the Interior, Bureau of Land Management conveying title and interest to the merchantable timber within a road easement across a county owned property.

2. Timber sale contract with Bruce Standley Construction for approximately 35,000 board feet of Douglas Fir.

PLANNING

1. Resolution approving and adopting the Douglas County Community Wildfire Protection Plan.

PUBLIC WORKS

1. Modification and fourth extension of contract with Orenco Systems Inc. for operation and maintenance of the Glide-Idleyld Park wastewater treatment facility and pressure sewer collection system.

2. Order accepting Bargain and Sale Deed from Promise Land Properties, Inc. for the acquisition of real property for the widening and realignment of South Calapooia Street.

SHERIFF

1. Order awarding contract to Talley Communications for microwave antennas and related equipment. Total purchase is $224,954.17.

2. Order awarding contract to Spectracom Corporation for simulcast common equipment for the communications project. Total purchase is $392,814.90.

3. Renewal of contract with AT&T for long distance services for the jail inmate phones. The Sheriff’s Department receives a commission from revenues generated from these calls. The commission percentage has increased from 25 to 30 percent retroactive to January 1, 2004.

MISCELLANEOUS

1. Order approving issuance of revenue bonds Series 2004A (Catholic Health Initiatives) by the Hospital Facility Authority of Umatilla County.

2. Roberts Creek Enterprise Zone sponsors agreement to extend property tax exemption to five consecutive years for capital investment by Umpqua Dairy Products Co.
