MINUTES of the board meeting of August 25, 2004 to be approved.
CITIZEN PARTICIPATION - Citizens wishing to address the commissioners concerning this agenda, or items of interest not included on this agenda, may do so at this time. When recognized, anyone addressing the commissioners will go to the microphone and give your name and address for the record.
ASSESSMENT AND TAXATION
1. Orders refunding taxes to the following: Lewis & Sons Logging, $741.88; Seward & Arlene Gulick, $495.30; James Lee and Al Young, $369.03; Grifform Innovations, Inc. $1,089.48; and Bradley Bremer, $388.37.
BUILDING
1. Resolution to apply for the electrical program from the State Building Codes Division.
CLERKS
1. Extension of contract with Ryder Election Services for ballot printing services. Contract amount not to exceed $145,000.
HEALTH AND SOCIAL SERVICES
1. Modification of supplemental terms and conditions with CHG Medical Staffing, Inc. for temporary contract for a Nurse Practitioner (Ada Hendricks). Agreement is for the period of August 16, 2004 through December 31. 2004, and amount is not to exceed $85,000.
2. Agreements with Glendale School District, Reedsport School District, and Riddle School District for Douglas County Health and Social Services to provide mental health treatment in the schools.
3. Supplemental provisions to contract with Mercy Medical Center, Inc for child and adult psychiatry services. Contract not to exceed $250,000 for the time period of September 2004 to September 2005.
JUVENILE
1. Acceptance of Juvenile Accountability Incentive Block Grant Award in the amount of $43,260 for fiscal year 2004-2005 for Juvenile Intake and Adjudication Project. Grant will continue to support a deputy district attorney to prosecute only juvenile delinquency cases. Grant requires a $37,387 cash match.
PUBLIC WORKS
1. Order accepting quit claim deed from the Bureau of Land Management for the acquisition of real property for the widening and realignment of South Calapooia Street.
