MINUTES of the board meeting of May 5, 2004 to be approved.

CITIZEN PARTICIPATION - Citizens wishing to address the commissioners concerning this agenda, or items of interest not included on this agenda, may do so at this time. When recognized, anyone addressing the commissioners will go to the microphone and give your name and address for the record.

PROCLAMATION declaring the week of May 16-22 as “Emergency Medical Services Week” in Douglas County.

BID OPENING

1. Request for statements of qualifications for architectural and related consultant services for replacement of the community center at the fairgrounds.

HEARING

1. Ordinance No. 2004-05-01. Request by Duane Brown for an amendment to the Douglas County Comprehensive Plan Map.

ASSESSMENT AND TAXATION

1. Order refunding taxes to Paul and Eileen Burgess in the amount of $601.50.

HEALTH AND SOCIAL SERVICES

1. Ambulance franchise agreements for emergency service contracts to six Douglas County ambulance service providers in eight service areas. Agreements will terminate January 31, 2009.

2. Contract between Douglas County Health and Social Services and Douglas County Association for Retarded Children (ARC) in the amount of $102,691. Clients who were once served through the Disability Service Office, will now be served through Developmental Disabilities Services Comprehensive In-Home Support Services.

3. Intergovernmental contract with the State of Oregon, which provides funding for the local public health authority to perform required environmental health services.

HUMAN RESOURCES

1. Order appointing Becky Mendenhall to fill the vacancy on the Employee Benefits Committee left by Anita Daily.

PARK

1. Amendment to original State Marine Board grant agreement for replacement of boarding floats and self-adjusting guide rails at Chief Miwaleta Park (Galesville Reservoir). This increases funding to pay for replacement of the second rail/float system. This will increase grant from $44,500 to $55,975.

2. State Marine Board facility grant agreement for installation of steel piling to install boarding floats at Scottsburg Park. This grant will provide all anticipated funding of this project up to $25,000.

PLANNING

1. Orders reappointing Kathy Holcomb to the Douglas Planning Advisory Committee, and Roger Drescher to the Elk Creek Planning Advisory Committee. Terms to expire May 12, 2007.

PUBLIC WORKS

1. Purchase of herbicides from UAP Timberland LLC from the Oregon Cooperative Purchasing Program. Total purchase is $31,373.

2. Third amendment of intergovernmental agreement for construction and transfer of jurisdiction of Lookingglass Road to the City of Winston.

3. Extension of contract with Umpqua Research Company to provide water quality analysis services to Douglas County. Contract amount is $26,000 per fiscal year through June 30, 2005.

SHERIFF

1. Extension of contract with Dr. James Olson to provide medical services to Sheriff’s Office as board certified pathologist to act as the county medical examiner in accordance with the provisions of Oregon Revised Statutes. Contract amount is $3,000 per month.

2. ATV grant agreement for enforcement of ATV laws and regulations and patrol of recreational areas including Oregon Dunes. Grant amount is $109,711. Total project cost is $154,678.

MISCELLANEOUS

1. Order reappointing Tom Michaels to the Douglas County Farm Board of Review. Term to expire June 30, 2006.
