MINUTES of the board meeting of December 22, 2004 to be approved.

CITIZEN PARTICIPATION - Citizens wishing to address the commissioners concerning this agenda, or items of interest not included on this agenda, may do so at this time. When recognized, anyone addressing the commissioners will go to the microphone and give your name and address for the record.

HEALTH AND SOCIAL SERVICES

1. Modification of contract for personal services with Olin and Associates, Inc. for practice management and billing software application development within the DMHC computer system. This will increase amount of contract from $25,000 to not exceed $50,000.

2. Order approving qualified mental health specialists to serve as Mental Health Director’s Designees.

HUMAN RESOURCES

1. Agreement with BCA (Best Choice Administrators) to provide third party administration of a Flexible Speeding Account Plan for Douglas County employees.

LAND

1. Real estate option and order to accept deed from Lori J. Vandervort for the acquisition of right of way to be used for culvert replacement on Roberts Creek Road. Consideration is $550.

2. Real estate option and order to accept deed from David and Wendy Barnum for the acquisition of right of way to be used for culvert replacement on Olalla Road. Consideration is $500.

PARK

1. Agreement with Lighthouse Gallery & Gifts for Lighthouse and Coast Guard Museum Concessionaire. Agreement amount is $650 per month paid to the county.

PUBLIC WORKS

1. Lease agreement with Elaine Reding for rock quarry site. Consideration is $1,100 per year or royalty at the rate of $0.50 per cubic yard, whichever is greater, for rock products removed by the county.

2. Real estate option and order accepting a warranty deed from Meredith Fenton, Barbara Fenton, Carlan Bratton Jr., Irene Bratton, Richard Forney, and Sharon Forney for the acquisition of real property for the widening and realignment of South Calapooia Street. Consideration is $4,527.



3. Real estate option and order accepting a warranty deed from Meredith Fenton, Barbara Fenton, Carlan Bratton Jr., and Irene Bratton for the acquisition of real property for the widening and realignment of South Calapooia Street. Consideration is $5,473.

SHERIFF

1. Local Law Enforcement Block Grant awarding and certifications for 20 Panasonic GPS/AVL systems for CF-29 Toughbook series, 4 Motorola HT 1250 portable radios, and 2 Dell desktop computers.

MISCELLANEOUS

1. Order appointing Blaine Nisson (President, Umpqua Community College) as an ex-officio member to the Douglas County Industrial Development Board.

2. Order reappointing Patricia Rhode and Betty Mack to the Douglas County Historic Resource Review Committee. Term to expire December 23, 2008.
