4. COMMISSIONERS – Appointment of Pro-Tem Judges (Paul Meyer)
The appointment of Drain District Justice of the Peace Pro-Tempore for 2017.

5. LAND DEPARTMENT – Order to Accept Sale Agreement & Addendum & Warranty Deed (Kevin Potter)
Order to accept Sale Agreement of County owned property located at 530 Portland Avenue, Roseburg. Offer was received from George D. Dawson and tentatively accept at $149,500. Kevin Potter has delegated authority to sign all required closing documents with First American Title Company of Oregon. Warranty Deed transferring ownership from Douglas County to George Dawson for the sale price of $149,500.

6. LAND DEPARTMENT – Second Modification of Lease (Kevin Potter)
Lease between Douglas County, Lessor and Family Relief Nursery of North Douglas, Lessee, dated January 14, 2015 with first modification recorded on August 10, 2016 extending the lease to December 31, 2016. This second modification extends the lease another year to December 31, 2017 and changes the Administrator to the Land Department. Lessee pays $150 per month.

7. MUSEUM – Museum Space Lease Agreement (Paul Meyer)
Museum space lease agreement and supplemental terms with Jessica Smalley dba Umpqua Edventure Academy. The lease is for a space at the museum to conduct a preschool.

8. PARKS DEPARTMENT – Contract Signing for Half Moon Bay Campground, Phase II (Rocky Houston)
Contract signing for Half Moon Bay Campground, Phase II. I.E. Engineering was the winning bid.

9. PUBLIC HEALTH – Funding Arrangement for Public Health and Umpqua Community Health Center (Dr. Bob Dannenhoffer)
This is a funding arrangement for reproductive health, immunization and sexually transmitted infections, not to exceed the amount of $196,354 for July 1, 2016 through June 30, 2017.

10. PUBLIC HEALTH – Seventh Amendment of Contract #27702 (Dr. Bob Dannenhoffer)
Funding arrangement between Douglas County and UCHC to provide school based health center at Douglas High School not to exceed the amount of $49,715 for July 1, 2016 through June 30, 2017.

Funding arrangement between Douglas County and UCHC to provide school based health center at Roseburg High School not to exceed the amount of $49,715 for July 1, 2016 through June 30, 2017.

Contract for a five year agreement for the acquisition of body cameras and Tasers, to include evidence storage and maintenance. The detailed contract has been reviewed and approved by County Counsel.

22. ADJOURNMENT AND NEXT MEETING

The next regularly scheduled Board meeting will be held December 28, 2016 at 9:00 a.m. in Room 216 of the Douglas County Courthouse, Roseburg, OR. The meeting is available via Video Conferencing to Reedsport citizens at the Reedsport Annex, 680 Fir Street, Reedsport, OR.

 

Douglas County may audio/video record the proceedings of this meeting for future broadcast or other purposes. A copy of any audio/video recording is available upon written request made to the Information Technology department. Cost is $5 per cd/dvd copy plus $2 shipping costs. Send a money order to: Douglas County Information Technology, 1036 SE Douglas, Room 123, Roseburg, OR 97470. Please include dates of requested dvd's or cd's and shipping address.
