CITIZEN PARTICIPATION FOR COMMENTS ON AGENDA ITEMS ONLY
1. PUBLIC HEARING – Annexation of Property (Keith Cubic)
Annexation of property and Order Incorporating Territory to the Tenmile Rural Fire District. Chief Petitioner: BYERS
2. PUBLIC HEARING – Annexation of Property (Keith Cubic)
Annexation of property and Order Incorporating Territory to the North Douglas County Fire and EMS. Chief Petitioner: RAWSON
3. BUILDING FACILITIES – Contract Award (Kevin Potter)
Order awarding Contract for the Health Center Elevator Modernization project. Cost: $282,368.00.
4. FLEET SERVICES – Order Authorizing Purchasing Vehicle (Mike Blank)-- POSTPONED
Order authorizing the purchase of one (1) new, full size, four wheel drive, four door police sport utility vehicle from Ron Tonkin Chevrolet under the Oregon State Price Agreement. Cost: $29,842.00.
5. HEALTH AND SOCIAL SERVICES – Agreement (Peggy Madison)
Agreement with Oregon Health Management Services, Inc. (OMHS) as part of the CCO plan. Agreement allows the Health Department to bill for services provided to clients assigned to OMHS for CHP care.
6. HEALTH AND SOCIAL SERVICES – Agreement Extension (Peggy Madison)
Order to extend Ambulance Service Franchise Agreements for all AREAS for an additional five (5) years.
7. PUBLIC WORKS – Order Accepting Permanent Easement (Robb Paul)
Order accepting Permanent Easement with Douglas County, Inc. for the Acquisition of Real Property for the widening and realignment of Del Rio County Road No. 115, Douglas County, Oregon.
8. MISCELLANEOUS – Appointment Order (Peggy Madison)
A vacancy exists on the Veterans Advisory Committee (VAC) due to the resignation of Mark Garcia. The VAC recommends the Appointment of Jerry Anderson in a term ending December 31, 2014.
9. MISCELLANEOUS – Reappointment (Susan Morgan)
A vacancy will exist on the Roaring Camp Road District due to the expiring term of Rosanna Hicks. Ms. Hicks would like to be reappointed with a term ending December 31, 2015.

NOTE: THERE WILL BE NO MEETING ON DECEMBER 26. The next regularly scheduled Board meeting will be held January 2, 2013 at 9:00 a.m. in Room 216 of the Douglas County Courthouse, Roseburg, OR.
