MINUTES of the board meeting of November 10, 2004 to be approved.

BID OPENING

1. Bid No. 877 for five, new ton, two wheel drive pickups, with long, wide box and standard cab.

HEARINGS

1. Annexation of property to the Douglas County Rural Fire Protection District No. 2. Chief Petitioner is Ruth Morgan.

2. Annexation of property to the Douglas County Rural Fire Protection District No. 2. Chief Petitioner is Clayton Judd.

3. Annexation of property to the Douglas County Rural Fire Protection District No. 2. Chief Petitioners are John and Patricia Miner.

4. Annexation of property to the Douglas County Rural Fire Protection District No. 2. Chief Petitioners are Chauncey and Jean Jennings.

5. Annexation of property to the Douglas County Rural Fire Protection District No. 2. Chief Petitioner is Linda Belding-Sheridan.

6. Annexation of property to the Douglas County Rural Fire Protection District No. 2. Chief Petitioners are Raymond and Marlene Grant.

FLEET SERVICES

1. Order rejecting all bids for Bid No. 876 for two, new mid-size, four door sedans. The Oregon State price agreement is $373.55 per unit under lowest responsive bidder.

2. Purchase agreement with Murray Chevrolet through the Oregon State Cooperative Purchasing Program for two, four door sedans. Purchase price is $30,195.90.

HEALTH AND SOCIAL SERVICES

1. Sublease contract with Patty Feola for office space located at 316 West “A” Street in Drain. Sublease is subject to all of the terms and conditions of the underlying lease between Dr. William Wiltse and Douglas County.

JUSTICE OF THE PEACE

1. Order appointing Russell Trump, Candace Hissong, Jad Lemhouse, Cindy Cable and/or John D. Cable as Reedsport Justice of the Peace Pro Tempore for the time period of September 22, 2004 through September 21, 2005.

PARK

1. License to use River Forks Park for the 12th annual Roseburg Rotary Festival of Lights display. The county provides labor and will receive 20 percent of the net revenues.

2. Grant application for $155,000 to improve the new Lighthouse ATV Staging Area. This grant requests funding for paving, double vault restrooms, and two structures that will serve as equipment storage and Dune Emergency Response office and meeting place.

PLANNING

1. Order approving boundaries and notice setting final hearing date of December 22, 2004 for annexation of property to the Green Sanitary District. Chief Petitioners are Clayton and Jessica Ruble.

2. Orders appointing Roger Johnson, George Seonbuchner, and Judith Bacon to the Calapooya Planning Advisory Committee. Terms to expire November 24, 2007.

PUBLIC WORKS

1. Resolution for the acquisition of real property for the reconstruction of a portion of Main Street in Canyonville.

2. Resolution for the acquisition of real property for the reconstruction of a portion of Little Valley Road.

3. Contract change order No. 4 with C2S Group, LLC in the amount of $37,760.95 for constructing a left turn lane into Dakota Street.

4. Contract change order No. 2 with C2S Group, LLC, in the amount of $21,930.09 for reconstruction of intersection of Highway 138 at Fort McKay Road.

5. Intergovernmental agreement with Umpqua Basin Watershed Council, Inc. in the amount of $25,000 for culvert replacement on Fate Creek.

6. Intergovernmental agreement with Umpqua Basin Watershed Council, Inc. in the amount of $20,000 for replacement of culvert on Rattlesnake Creek.

7. Intergovernmental agreement with the City of Sutherlin in the amount of $10,000 for Sutherlin Area Stormwater Modeling study.

8. Preliminary order approving vacation of a portion of Champagne Court without a hearing. Petitioner is Terry L. True.

9. Preliminary order approving a vacation of a portion of Melrose Road without a hearing. Petitioners are George and Alice Marie Crocker, Patsy Berry, Donald & Carol Vredenburg, and Knute Kershner.

10. Order accepting easement from Sam & Rebeccah Dunnavant for connection to the Glide-Idleyld Park sewer system.

SHERIFF

1. Contract with Douglas Electric Cooperative, Inc. to provide electric power to tower site in Elkton.

2. Order awarding contract to Day Wireless Systems for radio installation for the emergency communications project. Total purchase amount is $927,683.
