MINUTES Approval of the Minutes of October 29, 2014.

1. PUBLIC HEARING – Re-Annexation of Property (Keith Cubic)
Re-Annexation of Property and “Order Incorporating Territory” to Douglas County Fire District No. 2. Chief Petitioner: Lemings.

2. PUBLIC HEARING – Re-Annexation of Property (Keith Cubic)
Re-Annexation of Property and “Order Incorporating Territory” to Douglas County Fire District No. 2. Chief Petitioner: Miller.

3. PUBLIC HEARING – Re-Annexation of Property (Keith Cubic)
Re-Annexation of Property and “Order Incorporating Territory” to Douglas County Fire District No. 2. Chief Petitioner: Soehnlein.

4. HEALTH AND SOCIAL SERVICES – Lease Modification (Dawnelle Marshall)
First modification of lease between Canyonville, LLC and Douglas County to extend lease to November 30, 2014 for Public Health services. Cost: $1,070.11 per month.

5. HEALTH AND SOCIAL SERVICES – Contract Amendment (Dawnelle Marshall)
Amendment to UNI/CARE Agreement #22960. Reduces user count to 20 resulting in a reduced maintenance fee of $875.20 per month.

6. HUMAN RESOURCES – Contract Extension (Michael Kurtz)
Extension of contract with TriStar Risk Management for administration of workers’ compensation claims. Extends term by one year to November 8, 2015.

7. PUBLIC WORKS – Contract Award (Robb Paul)
Contract award of engineering services for Gardiner Wastewater Collection System Design and Construction to The Dyer Partnership, Engineers & Planners, Inc.
Contract award amount: $358,500.00

8. MISCELLANEOUS – Reappointment to the Solid Waste Advisory Committee (Robb Paul)
A vacancy will exist on the SWAC due to the expiring term of Brian Davis. The position was advertised with Douglas County media. Mr. Davis would like to be reappointed to a new term that would end January 1, 2019.

The next regularly scheduled Board meeting will be held November 12, 2014 at 9:00 a.m. in Room 216 of the Douglas County Courthouse, Roseburg, OR.

 

Douglas County may audio/video record the proceedings of this meeting for future broadcast or other purposes. A copy of any audio/video recording is available upon written request made to the Information Technology department. Cost is $5 per cd/dvd copy plus $2 shipping costs.
