MINUTES of the Board meeting held on October 12, 2011
CITIZEN PARTICIPATION FOR COMMENTS ON AGENDA ITEMS ONLY
1. BID OPENING Public Works (Robb Paul)
Emergency reconstruction of Bridge Rail Bridge 19C040, South Myrtle Road, County Road #18, mile post 9.20, Douglas County, Oregon.
2. FLEET SERVICES Bid Award #011 (Mike Blanck)
Award of Bid #011 for one (1) new half ton, four door, four wheel drive special service sport utility vehicle. The successful bidder is Clint Newell Auto Group of Roseburg, Oregon in the total amount of $30,557.80.
3. FLEET SERVICES Bid Award #012 (Mike Blanck)
Award of Bid #012 for three (3) new Ύ ton, four wheel drive pickups with short, wide box and crew cab. The successful bidder is Chrysler Jeep Dodge of Wilsonville Oregon in the total amount of $71,883.50.
4. FLEET SERVICES Bid Award #013 (Mike Blanck)
Award of bid #013 for one (1) new full size, half ton, four wheel drive pickup with short wide box and extended cab. The successful bidder is Chrysler Jeep Dodge of Wilsonville, Oregon in the amount of $22,388.88.
5. HEALTH AND SOCIAL SERVICES Order Authorizing Purchase of ADA Vans from Western Bus Sales, Inc. (Peggy Madison)
Order for the purchase of four (4) Special Transportation vans from Western Bus Sales, Inc. Purchase is made under Oregon State Contract #9705 and is exempt per LCRB Rule 4.24. Funding received via ODOT Grant. Cost is $46,636.00 per van for a total cost of $186,544.00.
6. HUMAN RESOURCES AFLAC Payroll Account Acknowledgement (Mike Hereford)
The AFLAC Payroll Account Acknowledgement Agreement changes the existing AFLAC payroll withholding payment from the current 28 day billing cycle to a monthly billing cycle and from 26 payroll deductions to 24 payroll deductions. The agreement also includes the addition of three new products for employee purchase; Accident Insurance, Critical Illness, and Hospital indemnity.
7. PLANNING Delegation of Authority Report (Keith Cubic)
Third quarter report of actions taken in the Planning Department on behalf of the Board of Commissioners.
8. PUBLIC WORKS Order Accepting Completion of Contract (Robb Paul)
Order accepting completion of the Slurry Seal of Winchester Bay Streets, Douglas County Roads #87B, 87C, 87D, 87E, 87F, 87G and 87H. The contract has been completed and all requirements have been met. Asphalt Maintenance Associates, Inc.: $38,680.70
9. PUBLIC WORKS Order Accepting Completion of Contract (Robb Paul)
Order accepting completion of overlay on Tiller Trail Highway, County Road #1, mile post 26.56 to mile post 34.96. The contract has been completed and all requirements have been met. LTM, Inc. dba Knife River Materials: $1,288,393.91.
10. PUBLIC WORKS Sutherlin Industrial Park Business Development Project (RobbPaul)
Grant Award from Business Oregon in the amount of $39,529.00 for the design plans for the Sutherlin Industrial Park Infrastructure improvements. Project funded in part with Oregon State Lottery funds administered by the Oregon Business Development Department. The grant funds are 85% of the estimated project cost.
11. PUBLIC WORKS Underground Right of Way Easement (Robb Paul)

12. PUBLIC WORKS Order with Western Federal Lands Highway Division (Robb Paul)
Order with Western Federal Lands Highway Division for ratifying FHWA Agreement No. DTFH70-11-E-00033 for the Tiller Trail Highway Pavement Preservation Project. The cost of work requested from the Forest Highway Program will not exceed $1,298,339.00.
13. MISCELLANEOUS Order Appointing Members to the Senior Services Advisory Council (Peggy Madison)
Two vacancies exist on the Senior Services Advisory Committee. The positions were advertised with Douglas County media and three applications were received and interviewed. The Council recommends the appointment of Loretta Crosier (position #9 representing North County) and Robert Grant (position #14 representing Nutrition/Meals program. Both terms will expire June 30, 2012.
14. MISCELLANEOUS Order Appointing Member to the Emergency Medical Services Advisory Committee (Peggy Madison)
A vacancy exists on the Emergency Medical Services Advisory Committee due to the resignation of Bill Duncan. The position was advertised with Douglas County media and one application was received. The Committee recommends the appointment of Mike Lane (current Roseburg Fire Department Chief) to a term that ends November 30, 2013.
15. MISCELLANEOUS Order Reappointing Member to Terrace Drive/Upper Terrace Drive Special Road District
A vacancy exists on the Terrace Drive/Upper Terrace Drive Special Road District due to the expired term of Dennis Bragg. Mr. Bragg would like to be reappointed to a new term that would expire December 31, 2013.
