MINUTES - Approval of the Minutes of September 23, 2015.

CITIZEN PARTICIPATION ON AGENDA ITEMS ONLY

1. BID OPENING Parks Department (Gary Groth)
Opening of Bids received for Bid #076 Parks Storage and Office Building Construction.

2. BID OPENING Salmon Harbor Department (Paul Stallard)
Opening of Bids received for Bid #075 One (1) New Ton 4x2 V8 Double Cab Standard Bed Pickup.

3. JUSTICE COURTS Contract (Paul Meyer)
Contract for legal defense services for indigent persons in the justice courts of Drain, Canyonville and Glendale. Contractor shall be paid $200.00 per case and the total contract amount is not to exceed $90,000.00 for a three year term.

4. PUBLIC HEALTH DEPARTMENT Contract Amendment (Dawnelle Marshall)
Fifth Amendment of Contract #27703 with Umpqua Community Health Center for operation of the School Based Health Center at Roseburg High School. Extends Contract to June 30, 2016. Contract amount: $50,350.00.

5. PUBLIC HEALTH DEPARTMENT Contract Amendment (Dawnelle Marshall)
Fifth Amendment of Contract #27702 with Umpqua Community Health Center for operation of the School Based Health Center at Douglas High School. Extends Contract to June 30, 2016. Contract amount: $50,350.00.

6. PUBLIC HEALTH DEPARTMENT Contract Amendment (Dawnelle Marshall)
CaCoon Contract with Oregon Health Sciences University for public health nursing support of care coordination for children birth-21 who have special health care needs. Contract Dates: 10/01/2015 -09/30/2016. Contract Amount: $22,000.00.

7. PUBLIC HEALTH DEPARTMENT Lease (Tim Freeman)
Lease with United Community Action Network (UCAN) for use of the Drain Health Department office two days per month for the Women, Infants, and Children (WIC) Program.

8. PUBLIC HEALTH DEPARTMENT Lease (Tim Freeman)
Lease with United Community Action Network (UCAN) for use of the Roseburg Health Department building for the Women, Infants, and Children (WIC) program. Term: October 1, 2015 - December 31, 2015.

9. PUBLIC HEALTH DEPARTMENT Lease (Tim Freeman)
Lease for temporary use of the WIC van for WIC services.

10. PUBLIC WORKS DEPARTMENT Order Accepting Completion of Contract (Robb Paul)
In the matter of the Contract for Mildred Kanipe Park Campground, Order Accepting Completion of Contract. The Contract with Knife River Materials has been completed and all requirements have been met.

11. PUBLIC WORKS DEPARTMENT Order Accepting Completion of Contract (Robb Paul)
In the matter of the Contract for paving of Garden Valley Road, County Road #31 and 31A. The Contract with Knife River Materials has been completed and all requirements have been met.

12. PUBLIC WORKS DEPARTMENT Order Setting Hearing (Robb Paul)
Order setting hearing for October 28, 2015 at 9:00 a.m. in Room 216 in the matter of transferring jurisdiction of a portion of South Comstock Road from Douglas County to the City of Sutherlin. County Road No. 154A, from its intersection with West Central Avenue (M.P. 0.0) to its intersection with Page Avenue (M.P. 0.82).

The next regularly scheduled Board meeting will be held October 7, 2015 at 9:00 a.m. in Room 216 of the Douglas County Courthouse, Roseburg, OR.

 

Douglas County may audio/video record the proceedings of this meeting for future broadcast or other purposes. A copy of any audio/video recording is available upon written request made to the Information Technology department. Cost is $5 per cd/dvd copy plus $2 shipping costs.
