9. CLERK’S DEPARTMENT – Contract Extension (Patricia Hitt)
Fourth Extension of Contract No. 27227 with Simplifile LC. for e-recording services. Extends agreement date to September 30, 2016.

10. FLEET DEPARTMENT – Request to Authorize Purchase (Robb Paul)
Request for Board authorization for the purchase of two (2) new one ton 15-passenger vans. No bids were received from the solicitation request Bid No. 074; therefore the Oregon Cooperative Procurement Program will be used. Total cost: $54,801.12.

11. HUMAN RESOURCES DEPARTMENT – Contract Amendment (Michael Kurtz)
First Amendment to Contract No. 29369 with Corvel Corporation for third-party workers’ compensation claim administration services.

12. LIBRARY DEPARTMENT – Grant Award Ratification Order (Harold Hayes)
Order ratifying grant amount from the Whipple Foundation Fund of the Oregon Community Foundation to purchase 13 self-checkout systems. Grant application was submitted on 4/14/2015 requesting $29,250.00. Funds were received on 5/13/2015.

13. MUSEUM – Order Awarding Contract (Gardner Chappell)
Order to award contract for quote #2462 for fencing enclosure at Museum.

14. MUSEUM – Contract (Gardner Chappell)
Contract #29396 with Pacific Northwest Fence Co. to provide and install all materials and labor for fencing enclosure at the Douglas County Museum. Contract amount: $25,372.00. Term Dates: August 5, 2015 – June 30, 2016.

15. PUBLIC HEALTH DEPARTMENT– Contract (Dawnelle Marshall)
Contract with Douglas Education Service District to provide nursing services for children in public school systems with special health care needs. Contract amount: $83,144.00 for 1520 hours. Term Dates: July 1, 2015 - June 30, 2016.

16. PUBLIC WORKS DEPARTMENT – Purchase of Real Property and Order accepting Warranty Deed (Robb Paul)
Purchase agreement and order accepting warranty deed and temporary construction easement from Babcock Loving Trust for the widening and realignment of Lookingglass Road No. 47. Purchase price: $500.00.

17. PUBLIC WORKS DEPARTMENT – Contract Change Order (Robb Paul)
Contract Change Order to Contract #29162 with LTM, Inc. dba Knife River Materials for the reconstruction of Old Highway 99S – Shady to Highway 42 project. The Change Order increases the Contract amount by $53,635.95 to include payment for work performed by Contractor to excavate solid ledge rock that was encountered during the general excavation operation.

18. SENIOR SERVICES DEPARTMENT – Intergovernmental Agreement (Jeanne Wright)
Intergovernmental Agreement #149047 with State of Oregon, Department of Human Services to support Mental Health Services for mild to moderate depression in seniors and people with disabilities. Revenue: $88,796.00. Term Dates: July 1, 2015 – June 30, 2017.

19. SHERIFF’S DEPARTMENT – Contract Extension (John Hanlin)
Twelfth Extension of Intergovernmental Agreement for school resource deputy with Glide School District. Extends contract to June 30, 2016. Revenue: $17,245.08.

20. SHERIFF’S DEPARTMENT – Contract Modification (John Hanlin)
Modification of Contract with BLM for Law Enforcement Patrol Services. Law Enforcement Services for 10/1/14-09/30/15 - $90,463.00; Law Enforcement Services for 10/1/15 – 09/30/16 - $60,376.00; Law Enforcement Services for 10/1/16 – 09/30/17 - $63,395.00; Law Enforcement Services for 10/1/17 – 09/30/18 - $66,565.00.

21. SHERIFF’S DEPARTMENT – Intergovernmental Agreement (John Hanlin)
Intergovernmental Agreement with Oregon Department of Transportation for use of a Strategic Technology Reserve (STR). The Agreement arranges for non-emergency use for training using the State’s STR’s. The only cost associated would be funding the restocking of the cache of the STR’s after training is conducted.

22. MISCELLANEOUS – Local Public Safety Commission Appointment (Chris Boice)
One vacancy exists due to the retirement of Douglas Ladd. The vacancy has a term expiration of June 30, 2016. Committee recommends the Appointment.

25. ADJOURNMENT AND NEXT MEETING
The next regularly scheduled Board meeting will be held August 19, 2015 at 9:00 a.m. in Room 216 of the Douglas County Courthouse, Roseburg, OR.

A copy of any audio/video recording is available upon written request made to the Information Technology department. Cost is $5 per cd/dvd copy plus $2 shipping costs.
