PLEDGE OF ALLEGIANCE

MINUTES of the Board meeting held on May 22, 2013

CITIZEN PARTICIPATION ON AGENDA ITEMS ONLY

1. PUBLIC HEARING – ANNEXATION CORRECTION (Keith Cubic)
Correction of Douglas County Fire District No. 2 Annexation Legal Description as required by the Department of Revenue.

2. BUILDING FACILITIES – CONTRACT MODIFCATION (Kevin Potter)
Contract Modification with Limbwalker Tree Service to increase the cap from $25,000.00 to not to exceed $50,000.00. Limbwalker Tree Service provides tree services to several county departments and is currently responsible for maintaining the trees at this facility which includes the Elm in front of the Courthouse.

3. COMMISSION ON CHILDREN & FAMILIES – INTERGOVERNMENTAL AGREEMENT (Gillian Wesenberg)
The 2013—15 Medicaid IGA between the Early Learning Division and Douglas County allows for the reimbursement of allowable Medicaid administrative expenses up to $65,000.00. Douglas County must pay administrative fees and match 50% of the reimbursement it receives. Revenue: Up to $65,000.00. Expense: Up to $65,000.00.

4. COMMISSION ON CHILDREN & FAMILIES – INTERGOVERNMENTAL AGREEMENT (Gillian Wesenberg)
The 2013-15 IGA (Mixed Funds Agreement) between the Early Learning Division and Douglas County includes two year allocations for Healthy Start and possible funding for flexible funds (Great Start, Children Youth & Families, and Family Support Services). Revenue: $311,493.00.

5. HEALTH AND SOCIAL SERVICES – ORDER (Peggy Madison)
Order approving qualified Mental Health Specialists to serve as mental Health Director’s Designees.

6. HEALTH AND SOCIAL SERVICES – CONTRACT MODIFICATION (Peggy Madison)
First Contract Modification with V Consulting and Associates, Inc. for personal services work on Community Health Assessment Project, Staff Training, and Leadership Coaching and Consulting. Expense: Maximum payable $50,000.00

7. PARKS – REVOCABLE LICENSE (Gary Groth)
Second Modification to Umpqua River Lighthouse Park Revocable License to expand their existing cell phone tower equipment. Rent has been increased from $500.00 to $1000.00 per month for the additional footprint.

8. PLANNING – PAC REMOVALS (Keith Cubic)
As provided in the Committee Involvement Ordinance, six members of our PAC program are being replaced. Coastal, one; Roseburg/Douglas, three; and, South County, two. The action is recommended by the Committee for Citizen Involvement.


9. PUBLIC WORKS – CONTRACT AWARD (Robb Paul)
Contract with Jack Mathis Construction, LLC., for crane service as needed. Contract not to exceed $50,000.00

10. PUBLIC WORKS – CONTRACT EXTENSION (Robb Paul)
Seventh Contract Extension with C.F. Malm, LLC., for SCADA system at Galesville Dam.

11. PUBLIC WORKS – ENGINEERING CONTRACT SERVICES (Robb Paul)
Engineering services contract with C. F. Malm Engineers, LLC., for engineering and consulting services for Galesville Dam. Services less that $25,000.00 may be authorized by the Public Works Director. Services more than $25,000.00 must be authorized by the Board of County Commissioners.

12. PUBLIC WORKS – CONTRACT EXTENSION (Robb Paul)
Seventh Contract Extension with Umpqua Research Company, Inc., for landfill monitoring analyses.

13. PUBLIC WORKS – CONTRACT AMENDMENT (Robb Paul)
Fourth Contract Amendment with Jones & Stokes Associates, Inc. for mitigation monitoring and report of the Wildlife Safari Wetland Mitigation Site. Amends contract from $30,000.00 to $38,000.00.

14. MISCELLANEOUS – REAPPOINTMENT TO ROAD DISTRICT (Commissioner Robertson)
A vacancy exists on the Overlook-Scottview Road District due to the expired term of Bryan Kollen. Mr. Kollen would like to be reappointed to a new term that would end December 31, 2015.

17. ADJOURNMENT AND NEXT MEETING
The next regularly scheduled Board meeting will be held June 5, 2013 at 9:00 a.m. in Room 216 of the Douglas County Courthouse, Roseburg, OR.
