CITIZEN PARTICIPATION FOR COMMENTS ON AGENDA ITEMS ONLY
1. HEALTH Lease with the City of Reedsport
Lease with City of Reedsport for Senior Center space for a new term beginning July 1, 2011 through June 30, 2012. Cost: $296.00 per month, $3,552.00 per term.
2. HEALTH Lease with Glide Community Club
Lease with Glide Community Club for Senior Center space for a new term beginning July 1, 2011 through June 30, 2012. Cost: $545.00 per month, $6,540.00 per term.
3. HEALTH Lease with City of Riddle
Lease with City of Riddle for Senior Center space for a new term beginning July 1, 2011 through June 30, 2012. Cost: $775.00 per month, $9,300.00 per term.
4. HEALTH Lease with Yoncalla Parks & Recreation District
Lease with Yoncalla Parks & Recreation District for Senior Center space for a new term beginning July 1, 2011 through June 30, 2012. Cost: $304.00 per month, $3,648.00 per term.
5. INFORMATION TECHNOLOGY Software Support Contract
Douglas County uses Intergraph Corporation software to maintain its Graphical Information System used by the Assessor, Surveyor and Public Works Engineering Departments. This contract provides ongoing annual maintenance and upgrades to this software. The total cost for this contract is $28,824.00. Intergraph is the only provider for this service.
6. MANAGEMENT & FINANCE Supplemental Budget
Supplemental Budget adjustments for the Salmon Harbor Fund. A HUD Grant in the amount of $74,558.00 will be used for the new Marine Activity Center.
7. PARKS/BUILDING FACILITIES Contract with Siemens Industry, Inc.
Bid #003 was opened on March 22, 2011 by Board of Commissioners. Lowest responsible bid was received from Siemens Industry, Inc. Order Awarding Contract was signed on April 20, 2011. Cost to County: $114,840.00.
8. PUBLIC WORKS Farm Lease Extension
Farm Lease extension with Tanner Langdon for 273 acres of the Milltown Hill property.
9. PUBLIC WORKS Contract with Forslund Crane Service Inc.
Contract No. 26217 with Forslund Crane Service, Inc. for crane services as needed for the amount not to exceed $50,000.00. Base rate of $310.00 an hour with a 2 hour minimum plus mobilization as required.
10. PUBLIC WORKS Contract with Jack Mathis Construction LLC
Contract No. 26216 with Jack Mathis Construction, LLC for crane services as needed for the amount not to exceed $50,000.00. Base rate of $335.00 an hour with a 3 hour minimum plus mobilization as required.
11. PUBLIC WORKS Contract with Asphalt Maintenance Associates, Inc.
Contract with Asphalt Maintenance Associates, Inc. in the amount of $32,138.20 for Slurry Seal of Winchester Bay Streets County Roads No. 87B, 87C, 87D, 87E, 87F, 87G, 87H, Douglas County, Oregon.
12. PUBLIC WORKS Extension of Lease with Department of Transportation
Seventh Extension of Lease with the Department of Transportation for space located at 433 Rifle Range Road, Roseburg, Oregon. Revenue of $1,500.00 per month.
13. PUBLIC WORKS Extension of Agreement with Roseburg Refrigeration
Second Extension of Agreement with Roseburg Refrigeration for removal of CFCs from appliances at the Roseburg and Reedsport landfills. $45.00 an hour as needed.
14. PUBLIC WORKS First Modification of Contract with I.E. Engineering, Inc.
First Modification of Contract No. 25817 for with I.E. Engineering, Inc. for Del Rio Road Realignment Engineering Services.
15. PUBLIC WORKS Extension of Contract with Umpqua Research
Fifth Extension of Contract with Umpqua Research Company, Inc. for landfill monitoring analyses. Rates vary.
16. PUBLIC WORKS Lease with Susie L. Harms and John D. Brown III
Lease with Susie L. Harms and John D. Brown III for 292 acres of the Milltown Hill property.
17. SHERIFF Amendment for Wildlife Services
Contract Amendment to increase funding to Wildlife Services from $130,815.00 to $144,225.75. $13,410.75 increase.
18. MISCELLANEOUS Discussion/Deliberation on Appointments to the Veterans Advisory Committee
Two vacancies exist on the Veterans Advisory Committee due to resignations. The vacancies were advertised with Douglas County media and four applications were received. The applications were reviewed by the Veterans Advisory Committee on April 27th and recommend the appointment of Mark Garcia (Persian Gulf) with term ending December 31, 2012 and the appointment of Tim Rogers (Vietnam) with term ending December 31, 2011.
