1. PUBLIC HEARING – RE-ANNEXATION OF PROPERTY (Keith Cubic)
Public Hearing for the Re-Annexation of Property and “Order Incorporating Territory” to the Winston-Dillard Rural Fire District. Chief Petitioner: ANDREWS.

2. PUBLIC HEARING – RE-ANNEXATION OF PROPERTY (Keith Cubic)
Public Hearing for the Re-Annexation of Property and “Order Incorporating Territory” to the Azalea Rural Fire District. Chief Petitioner: BRADY.

3. PUBLIC HEARING – RE-ANNEXATION OF PROPERTY (Keith Cubic)
Public Hearing for the Re-Annexation of Property and “Order Incorporating Territory” to the Oakland Rural Fire District. Chief Petitioner: BOONE.

4. PUBLIC HEARING – RE-ANNEXATION OF PROPERTY (Keith Cubic)
Public Hearing for the Re-Annexation of Property and “Order Incorporating Territory” to the North Douglas County Fire and EMS. Chief Petitioner: FRANKLIN.

Public Hearing for the Re-Annexation of Property and “Order Incorporating Territory” to the Douglas County Fire District No. 2. Chief Petitioner: BURKE.

6. JUVENILE – AMENDED INTERGOVERNMENTAL AGREEMENT (Aric Fromdahl)
Amendment to an Intergovernmental Agreement with the State of Oregon, Department of Human Services and Douglas County for placement of youth in the Douglas County Shelter Care Facility. This amendment increases the average number of youth per day from four to six. Revenue: $129.94 per youth per day.

7. LIBRARY – DELEGATION OF AUTHORITY (Harold Hayes)
Delegation of Authority to Harold Hayes to claim beneficial interest in an annuity from ING. The money is from Anna Christine Helland’s annuity gift to the Douglas County Library System for the benefit of the Yoncalla Branch Library. Annuity was approximately $2,548.00 at the time of Ms. Helland’s death.

8. LAND – LEASE WITH CANYONVILLE, LLC. – (Gary Groth)
Lease between Douglas County and Lessor, Canyonville, LLC, for office space located at 247 and 249 N. Main Street, Pioneer Center, Canyonville. Lease will commence June 1, 2013 through May 31, 2016. Rent will remain at $1,070.11 per month per property for the first two years, with an option for increase dependent on market rental data.

9. PUBLIC WORKS – ORDER AUTHORIZING PURCHASE (Robb Paul)
Order authorizing the purchase of Snap-Tite Pipe from Isco industries. Cost: $27,661.59

10. PUBLIC WORKS – CONTRACT AMENDEMENT (Robb Paul)
Amendment Number 01 with Oregon Department of Transportation for Local Agency Agreement Emergency Relief Program, Eagle Valley Road (County Road #389).

11. PUBLIC WORKS – CONTRACT MODIFICATION (Robb Paul)
Second Contract Modification with I.E. Engineering, Inc. for Del Rio Road realignment engineering services.

12. MISCELLANEOUS – REAPPOINTMENT TO ROAD DISTRICT (Doug Robertson)
A vacancy exists on the Upper Cleveland Rapids Special Road District due to the expired term of Patsy Smith. Ms. Smith would like to be reappointed to a new term that would end December 31, 2015.

The next regularly scheduled Board meeting will be held May 29, 2013 at 9:00 a.m. in Room 216 of the Douglas County Courthouse, Roseburg, OR.
