PLEDGE OF ALLEGIANCE

MINUTES - Approval of the Minutes of May 13, 2015.

CITIZEN PARTICIPATION ON AGENDA ITEMS ONLY

1. PUBLIC HEARING, Continued – Remand of Nonfarm Dwelling (Keith Cubic)
The public hearing held in the matter on May 13, 2015 was continued for one week to a date of May 20, 2015 to allow for review of an amendment to the Order proposed by the applicant.

2. DINT – Order Awarding Contract (Pat Moore)
RFP#070 was opened on April 16, 2015 by Management & Finance. Most advantageous Proposal was received from Apex Heli Oregon, Inc.

3. LAND DEPARTMENT – State Offer & Warranty Deed (Gary Groth)
ODOT submitted an offer to purchase 2,841 square feet of property adjacent to the Harvard Avenue right-of-way located on the County’s ownership of 621 W. Madrone Street. ODOT is planning to improve traffic flow in the area by repositioning the bus stop structure away from Harvard Avenue and constructing a turnout so that a bus can pull off and stop without blocking Harvard avenue traffic. Purchase price: $31,700.

4. PUBLIC WORKS DEPARTMENT – Contract Modification (Robb Paul)
First modification of contract with the Dyer Partnership to include an Environmental Review Report. Increases contract $15,000 for a total contract amount of $373,500.00.

5. PUBLIC WORKS DEPARTMENT – Agreement (Robb Paul)
Agreement to pay Department of Environmental Quality (DEQ) a penalty assessment for violation of Roseburg Landfill stormwater permit. Cost: $1,440.00.

6. PUBLIC WORKS DEPARTMENT – Contract Award (Robb Paul)
Award and execution of Contract #29251 with LTM, Inc. dba Knife River Materials in the amount of $321,457.00 for the Paving of Garden Valley Rd, County Rd #31 & #31A project. Two bids for this project were received and opened at the regularly-scheduled Board of Commissioners meeting on April 29, 2015.

7. SENIOR SERVICES DEPARTMENT – Board Order (Jeanne Wright)
This is a Board Order granting Jeanne Wright the authority to review and approve routine matters in the Senior Services program, enter into agreements that do not exceed $25,000 in cost, and review agenda items related to Senior Services.

8. SENIOR SERVICES DEPARTMENT – Amendment (Jeanne Wright)
This is Amendment number 02 to Intergovernmental Agreement Number 141212 between the State of Oregon acting by and through its Department of Human Services and Douglas County Senior Services. This amends the ADRC infrastructure IGA to extend the term from 06/30/15 to 09/30/15 and adds $35,000.00 in additional revenue, for a total of $100,000.00.

9. SENIOR SERVICES DEPARTMENT – Lease Agreement (Jeanne Wright)
This is an annual Lease Agreement with Glide Community Club for Senior Dining space. Expenditure of $578.00 per month and term dates of 07/01/2015 to 06/30/2016.

10. SENIOR SERVICES DEPARTMENT – Lease Agreement (Jeanne Wright)
This is an annual Lease Agreement with City of Reedsport for Senior Dining space. Expenditure of $333.00 per month and term dates of 07/01/2015 to 06/30/2016.

11. SENIOR SERVICES DEPARTMENT – Lease Agreement (Jeanne Wright)
This is an annual Lease Agreement with Sutherlin Seniors, Inc. for Senior Dining space. Expenditure of $651.00 per month and term dates of 07/01/2015 to 06/30/2016.

12. MISCELLANEOUS – Appointments to the Planning Advisory Committee (Keith Cubic)
This is for reappointments to the Planning Advisory Committee; Two each for Callahan and North County. One each for Coastal and North Umpqua. This reappointment process is consistent with the Citizen Involvement Ordinance and honors active volunteers who have developed a working knowledge of the land use process. Reappointments are for three year terms, to expire May 20, 2018.

13. MISCELLANEOUS – Reappointments to CCD Business Development Corporation (Commissioner Boice)
Eight vacancies will exist on the CCD Board due to the expiring terms of Cameron Frasnelly, Sean Negherbon, Twila Rae Van Loon, Ron Doan, Cindy Kent, Joel Hooper, Carl Patenode and Chris Boice. All will to be reappointed to new terms that would expire May 31, 2016.

17. ADJOURNMENT AND NEXT MEETING
The next regularly scheduled Board meeting will be held May 27, 2015 at 9:00 a.m. in Room 216 of the Douglas County Courthouse, Roseburg, OR.
