2. Approval of the Minutes of May 11, 2016

4. LAND DEPARTMENT – Road Use Permit (Gary Groth)
Road Use Permit between Douglas County and Rosboro, LLC. Permit allows Rosboro, LLC, the non-exclusive right to enter a road system for transportation of forest products from nearby forest lands owned by Rosboro, LLC. Permit expires on December 31, 2021.

5. LAND DEPARTMENT – Permanent Access Easement (Gary Groth)
Approval of a perpetual non-exclusive permanent access easement between Douglas County and Dimmick Cemetery Association for cemetery access purposes. This easement is in the Elkton-Sutherlin area.

6. PUBLIC HEALTH – Medicaid Administrative Claim (Dr. Bob Dannenhoffer)
Public Health Medicaid Administrative Claim for the Summer Quarter 2015. The total Claim Amount is $16,900.50. The fifty percent match amount to be paid to Oregon Health Authority is $8,450.25.

7. PUBLIC HEALTH – Contract Modification (Dr. Bob Dannenhoffer)
Second Contract Modification with the Douglas County Public Health Network . Modification allots funds in the amount of $4,964.00 received from the Local Public Health Authority for Maternal and Child Health to Douglas Public Health Network.

8. PUBLIC HEALTH -- Contract Amendment (Dr. Bob Dannenhoffer)
Sixth Contract Amendment to the Local Public Health Authority with the State. The amendment makes small changes in the language related to Ebola expenditures, but has no financial impact on Douglas County.

9. PUBLIC WORKS – Contract (Robb Paul)
Contract with Forslund Crane Service, Inc. for crane services as needed. Cost is $330.00 per hour with a 4 hour minimum.

10. PUBLIC WORKS – Contract (Robb Paul)
Contract with Jack Mathis Crane Service for crane services as needed. Cost is $357.00 per hour with a 3 hour minimum.

11. REEDSPORT JUSTICE COURTS – Intergovernmental Agreement (Paul Meyer)
Intergovernmental Agreement authorized by ORS 156.315 and 137.118 for collection by the Oregon Department of Revenue on behalf of Reedsport Justice Court of delinquent court-ordered financial obligations in criminal and violation cases through offset of debtors’ state tax refunds. The Oregon Department of Revenue retains a collection fee of 10% of the amounts collected.

12. SHERIFF – Easement Agreement (Wayne Stinson)
Renewal of the Road Easement Agreement with Lone Rock Timber for access to the Lane Mountain Bureau of Land Management radio site.

13. SHERIFF – Extension of Intergovernmental Agreement (John Hanlin)
Twelfth extension of the Intergovernmental Agreement with the City of Winston for inmate housing. Cost: $86.88 per day.

14. SHERIFF – Extension of Intergovernmental Agreement (John Hanlin)
Twelfth extension of the Intergovernmental Agreement with the City of Myrtle Creek for inmate housing. Cost: $86.88 per day.

The next regularly scheduled Board meeting will be held May 25, 2016 at 9:00 a.m. in Room 216 of the Douglas County Courthouse, Roseburg, OR. The meeting is available via Video Conferencing to Reedsport citizens at the Reedsport Annex, 680 Fir Street, Reedsport, OR.
