MINUTES of the Board meeting held on May 8, 2013

CITIZEN PARTICIPATION ON AGENDA ITEMS ONLY

1. PUBLIC HEARING – RE-ANNEXATION OF PROPERTY (Keith Cubic)
Public Hearing for the Re-Annexation of Property and “Order Incorporating Territory” to the Calapooia Rural Fire District. Chief Petitioner: ZADEH

2. PUBLIC HEARING – RE-ANNEXATION OF PROPERTY (Keith Cubic)
Public Hearing for the Re-Annexation of Property and “Order Incorporating Territory” to the North Douglas County Fire and EMS District. Chief Petitioner: STRIBLING

3. BUILDING FACILITIES – CONTRACT EXTENSION (Kevin Potter)
Second Contract Extension with Otis Elevator Company to provide elevator maintenance services as per contract. Contract extends to April 27, 2014. Cost: $29,895.36.

4. COMMISSION ON CHILDREN AND FAMIIES – PROJECT CONTRACT FOR WINSTON AREA – (Gillian Wesenberg)
Contract with the Winston Area Community Partnership for the Build Our Kids Replication Project. The Contract has a maximum payable amount of $25,000 for the period of May 1, 2013 to June 30, 2014. Funding is provided from a grant received from the Ford Family Foundation.

5. COMMISSION ON CHILDREN AND FAMIIES – PROJECT CONTRACT for RIDDLE SCHOOL DISTRICT – (Gillian Wesenberg)
Contract with the Riddle School District for the Build Our Kids Replication Project. The Contract has a maximum payable amount of $25,000 for the period of May 1, 2013 to June 30, 2014. Funding is provided from a grant received from the Ford Family Foundation.

6. COMMISSION ON CHILDREN AND FAMIIES – PROJECT CONTRACT FOR PHOENIX SCHOOL – (Gillian Wesenberg)
Contract with Phoenix School for the Build Our Kids Replication Project. The contract has a maximum payable amount of $25,000 for the period of May 1, 2013 to June 30, 2014. Funding is provided from a grant from the Ford Family Foundation.

7. INFORMATION TECHNOLOGY – CONTRACT EXTENSION (Kevin Potter)
Second Contract extension with Intergraph, Inc., for the automated Geographic Information System (GIS) software. The software is used by the Assessor, Surveyor and Engineering Departments to maintain County maps. The total annual cost is $29,100.00 and is divided between the Assessor, Engineering and Surveyor Departments.

8. LAND – ORDER AND WARRANTY DEED – (Gary Groth)
Order ratifying the terms of sale of 4.69 acres of surplus land on Mulholland Drive to Dash Enterprises of Roseburg. Statutory Warranty Deed transferring ownership from Douglas County to Dash Property holdings, LLC for the sale price of $1,250,000.

9. PARKS – ORDER AUTHORIZING PURCHASE (Gary Groth)
Order authorizing the purchase of prefabricated restroom/shower from CXT, Inc. for Chief Miwaleta Park. Cost: $117,050.00.
10. PUBLIC WORKS – CONTRACT AWARD (Robb Paul)
Contract with Blackline, Inc. in the amount of $68,330.70 for Slurry Seal of Tri City Streets.

11. PUBLIC WORKS – EXTENSION OF CONTRACT – (Robb Paul)
First Contract Extension with Roseburg Refrigeration for removal of Chlorofluorocarbons from refrigerated units at Roseburg and Reedsport Transfer Sites. Cost: $45.80 per man hour.

12. SHERIFF – COMMUNICATIONS SERVICES AGREEMENT – (John Hanlin)
Agreement with Douglas County Fire District #2 for fire dispatching services. Cost is $56,396.72 per year for the 1st year with a 2-5% annual increase.

15. ADJOURNMENT AND NEXT MEETING
The next regularly scheduled Board meeting will be held May 22, 2013 at 9:00 a.m. in Room 216 of the Douglas County Courthouse, Roseburg, OR.

A copy of any audio/video recording is available upon written request made to the Information Technology department. Cost is $5 per cd/dvd copy plus $2 shipping costs.
