4. PUBLIC HEALTH Contract Amendment (Dr. Dannenhoffer)
Tenth amendment of Contract No. 27703 between Douglas County and Umpqua Community Health Center, to amend term date from June 30, 2018 to February 15, 2018.

5. PUBLIC HEALTH Contract Amendment (Dr. Dannenhoffer)
Fifth amendment of Contract No. 29525 between Douglas County and Umpqua Community Health Center, Inc., to modify term date from June 30, 2018 to February 15, 2018.

6. PUBLIC HEALTH Contract Amendment (Dr. Dannenhoffer)
Fourth amendment of Contract No. 29529 between Douglas County and ADAPT, to amend term date from June 30, 2018 to February 15, 2018.

First amendment of Contract No. 30782 between Douglas County and ADAPT, to amend term date from June 30, 2018 to February 15, 2018.

Fourth modification of Contract No. 29613 between Douglas County and United Community Action Network, to modify term date from June 30, 2018 to February 15, 2018.

10. PUBLIC HEALTH Contract Modification (Dr. Dannenhoffer)
Ninth modification of Contract No. 29674 between Douglas County and Douglas Public Health Network, to modify term date from June 30, 2018 to February 15, 2018.

11. PUBLIC HEALTH Contract (Dr. Dannenhoffer)
Contract No. (TBD) for new agreement between Douglas County and Douglas Public Health Network, to begin services February 16, 2018 through June 30, 2018.

12. ASSESSOR Roll Correction Refund (Roger Hartman)
Non Profit Entity applied for charitable exemption late with late filing fee. Partial Exemption applied to the account to reflect exempt portion, which will be separately allocated to the new account upon completion of roll correction. Acct # R27797, total refund with interest is $4,534.33.

13. ASSESSOR Roll Correction Refund (Roger Hartman)

14. COMMISSIONERS Appointment to the Parks Advisory Board (Commissioner Boice)
A vacancy exists on the Parks Advisory Board due to the resignation of Richard Patten. An Open Committee announcement for a Coastal Representative was sent to local media and posted on the County website. Three applications were received. Request appointment of Jody Morrow to assume the term expiring 12/27/2018.

15. COUNTY CLERK Contract Extension (Patricia Hitt)
Extension of Contract #29105 between Douglas County for Bullfrog Enterprises, Inc. to extend contract term until March 25, 2019.

16. COUNTY COUNSEL Ordinance Public Hearing (Paul Meyer)
Second reading of Ordinance No. 2018-01-02 authorizing a grant to Douglas Community Library Association. First reading was on January 31, 2018; with an effective date of May 21, 2018.

17. LAND DEPARTMENT Notice of Timber Sale (Keith Cubic)
Notice of Timber Sale for the 2018-2019 harvest year. Cedar Hollow Salvage #305 contains approximately 8 acres, 112,000 BF with an estimated value of $16,800. Scholfield Winchester #308 contains approximately 45.5 acres, 2,485,000 BF with an estimate value of $906,410.00. Both sales will be offered at oral auction on March 6, 2018 at 10:00am in Room 216.

18. PLANNING DEPARTMENT Reappointment to Emergency Medical Services Committee (Keith Cubic)
Gregg Timm has served on the EMSC for 3 years. He serves in a slotted position as required by the County ASA Ordinance. He represents the professional fire service. The EMSC Committee recommended at their meeting on November 4, 2017 that Gregg Timm be reappointed to the Committee.

19. PLANNING DEPARTMENT Appointment of member to Planning Commission (Keith Cubic)
A vacancy exists on the Planning Commission due to the resignation of Victoria Hawks whose term was to expire on February 28, 2018. Following an open recruitment two applications were received. Planning Commission membership is based on occupational diversity, geographic area, differeing interests and related experience.
Based on membership criteria and application review it is recommended that Virgile Osborne be appointed to the Planning Commission, the term to expire February 28, 2022.

20. PLANNING DEPARTMENT - Public Hearing (Joshua Shaklee)
Public Hearing and Order Calling for an Election by the affected property owners of the formation of the Lower Umpqua Library District and the approval of a maximum tax rate limit of $0.39 per $1000.00 of assessed value. First Hearing was held on January 17, 2018. Election is called for May 15, 2018.

Speakers will be limited to three minutes with a total of 30 minutes allocated for this portion of the meeting.

23. ADJOURNMENT AND NEXT MEETING
The next regularly scheduled Board meeting will be held February 21, 2018 at 9:00 a.m. in Room 216 of the Douglas County Courthouse, Roseburg, OR.
