3. PROCLAMATION – Douglas County School Choice Week (Commissioner Boice)

5. PUBLIC HEALTH – Contract Amendment (Dr. Dannenhoffer)
Third amendment to Contract #31863 between Douglas County and DPHN to provide an increase of funding in the amount of $1,968.00 for PE12 Public Health Emergency Preparedness & Response (PHEP); and an increase of funding in the amount of $25,943.00 for PER13-01 Tobacco Prevention & Education Program (TPEP).

6. LAND DEPARTMENT – Notice of Timber Sale (Rocky Houston)
Notice of Timber Sale for the 2020 harvest year. 2020 Park Hazard Tree Removal – Cavitt Creek Park PK #012 contains approximately 5 acres, 37.4 MBF with an estimated value of $5,000. Sealed bids will be due at the Land Department office by 10:00 a.m., on Wednesday, February 12, 2020. Sale expiration date is May 15, 2020.

7. LAND DEPARTMENT – Lease Agreement (Rocky Houston)
Lease agreement between Douglas County and Daniel and Samantha Campbell for grazing rights on approximately 1,000 acres at Mildred Kanipe Park. Lease term is from January 1, 2020 through December 31, 2025 with the option of one additional five-year extension. Rent for the first twelve months is $1,400/month and increases each year by 3%.

8. LAND DEPARTMENT – Lease Extension (Rocky Houston)
Fifteenth (15th) Extension of Lease between Douglas County and the State of Oregon for 4,000 rentable square feet of office space located in the Douglas County Public Works Department Complex at 433 Rifle Range Road, Roseburg, Oregon. Lease is amended to extend term from January 1, 2020 through June 30, 2020 with a new lease being prepared in the interim by DAS. Rent is $2,111.50 per month.

9. PARKS DEPARTMENT – Contract Modification (Rocky Houston)
Second modification of contract #31680 between Douglas County and i.e. Engineering to provide planning, engineering and design work for the Umpqua Dunes Campground – South Loop Project. This modification authorizes i.e. Engineering to perform Construction Administration on a Time and Material, not to Exceed Basis, of $15,600.00.

Public Hearing for Annexation of Property and “Order Incorporating Territory” to the Fair Oaks Rural Fire District (Chief Petitioner: BOLANDER)

Public Hearing for Annexation of Property and “Order Incorporating Territory” to the Douglas County Fire District No. 2 (Chief Petitioner: NOWA JOINT LIVING TRUST)

12. PLANNING – Grant Award (Joshua Shaklee)
Douglas County has been awarded a mini grant from the National Environmental Health Association and Centers for Disease Control to participate in the National Environmental Assessment Reporting System (NEARS). The award is in the amount of $2,500.00. The project period is from February 1, 2020 through June 30, 2020.

13. PLANNING – Intergovernmental Agreement (Joshua Shaklee)
Intergovernmental Agreement between Douglas County (Agency) and Lane County (County) to cover the cost of Agency using HealthSpace inspection software are services through authority granted to Lane County by Oregon Health Authority (OHA). The term of contract is from January 1, 2020 through December 31, 2020.

14. PLANNING – Contract Agreement (Joshua Shaklee)
Contract #30000176 between Douglas County and US Forest Service, dba J. Herbert Stone Nursery, for the purchase and growing of approximately 46,000 Douglas-fir 1+1 and approximately 14,000 Douglas-fir 2-0 seedlings to be delivered by February 2022. Total cost of the finished seedlings will be $27,410.00.

15. PLANNING – Emergency Medical Services (EMS) Advisory Committee Appointment (Joshua Shaklee)
Recommendation that Undersheriff Jeff Frieze be appointed to the Emergency Medical Services (EMS) Advisory Committee in the position of law enforcement officer (as designated in Ambulance Service Ordinance Section 8.10.210(E)(5)).

16. PUBLIC WORKS/Fleet – Order Authorizing Purchase (Scott Adams)
Order authorizing purchase of two (2) 2020 Ford Explorer Sport Utility 4-Door 4X4 vehicle from Landmark Ford of Tigard, Oregon. Douglas County is authorized to purchase from the State Price Agreement Contract #5550 and this purchase is exempt from competitive bidding. Purchase price $30,059.25 each for a total of $60,118.50.

17. PUBLIC WORKS – Permit of Entry (Scott Adams)
Permit of Entry with Kenneth and Cindy Makinson allowing the County to enter their property to perform activities required to install and maintain sidewalk along Old Hwy 99N (County Rd. #388) at milepost 12.36 as part of the Reconstruction Project of Old Hwy 99N.

18. PUBLIC WORKS – Order Authorizing Purchase (Scott Adams)
Order authorizing purchase of three (3) 2021 Kenworth T800 trucks, 15’ Dump box, 10’ Plow blade, and Sander box from National Auto Fleet Group. Douglas County is authorized to purchase from the Sourcewell contract #081716-NAF and this purchase is exempt from competitive bidding. Purchase price of $206,116.00 each, for a total of $618,348.00.

19. SHERIFF’S DEPARTMENT – Order Authorizing Purchase (Undersheriff Frieze)
Order authorizing purchase of a 2020 YXZ 1000R-XTR Turbo UTV using funding received from the ATV Grant Agreement in the amount of $26,076.00.

20. SHERIFF’S DEPARTMENT – Order Authorizing Purchase (Wayne Stinson)
Order authorizing purchase of night vision equipment from TNVC Inc. in the amount of $28,841.00.
