If you have never had a passport you will need to apply at the Douglas County Clerk's Office in person.

Please bring:

• Certified copy of your Birth Certificate issued by the vital records division of the state where you where born or previous US Passport. Your Birth Certificate will be sent along with your application and returned to you.
• Your original Naturalization Certificate (if you are a naturalized citizen). It will be sent along with your application and returned to you.
• A Certificate of Birth Abroad if you were born to U S parents abroad. It will be sent along with your application and returned to you.
  

1. The clerk's office now has the ability to take passport photos. The cost is $15 and available from 9 a.m. to 4 p.m. when the office is open. Or you can bring two identical photos that have been taken within the last 6 months. The photos must be 2" X 2" and have a white or off-white background. Travel Agencies and professional photographers can take passport photos for you. School photos are not acceptable.
   
   
2. A valid Driver's License or Government issued ID. Motor Vehicle ID cards are acceptable only with one other piece of ID or supporting identifying witness affidavits.
   
   
3. A completed DS11 application (available at our office or online at Department of State Website)

TRAVEL DOCUMENTS FOR U.S. CITIZENS - Apply for one or both, it’s your choice.

U.S. PASSPORT BOOK - Valid for travel by air, sea or land.

Validity:

• 10 years for adults
• 5 years for minors

• $100 for adults
• $85 for minors

U.S. PASSPORT CARD - Not for travel by air. Valid for land and sea crossings between the U.S. and Canada, Mexico, Bermuda and the Caribbean only.

Validity:

• 10 years for adults
• 5 years for minors

• $45 for adults
• $35 for minors

If you apply for both documents at the same time -

Cost for first time applicants:

• Adults: $120
• Minors: $95
