Small image of a US PassportA Passport book will take you anywhere in the world you want to go by land, sea or air. A Passport card can only be used to travel between Canada or Mexico and the U S by land or sea or to travel by sea to the Caribbean area. You cannot board an airplane to leave the U S or to return to the U S using a Passport card.

Anyone of any age, who has never had a Passport book or Passport card or who has an adult Passport book that was issued more than 15 years ago must apply in person at the Douglas County Clerk’s Office at the Courthouse in Roseburg or at the Douglas County Annex in Reedsport. Please remember that neither of these offices accept credit or debit cards and are not allowed to send cash to U S Passport Services. See below under “Fees” for acceptable methods of payment.

Passport applications are accepted 8:00 am to 4:00 pm Monday thru Friday, except holidays, in the Douglas County Clerk’s Office, Room 124 of the Douglas County Courthouse in Roseburg. Our phone number is 541-440-4320. Applications are also accepted 8:00 am to 4:00 pm Monday thru Friday, except holidays, at the Douglas County Annex in Reedsport. The phone number is 541-271-3641.

An adult (16 years old or older) Passport book or Passport card is valid for 10 years.
A child’s (15 years old or younger) Passport book or Passport card is valid for 5 years.

Normal processing time is 4 to 6 weeks. If you need it faster, refer to “Expediting” below.

 

Applying for a Passport:

To apply for a first time Passport book or Passport card you must bring one of the following:

**A certified copy of your Birth Certificate issued by the vital records division of the state where you were born. Please make sure your Birth Certificate has both of your parents names listed on it. If it does not, you will need to get a new certified copy from the state where you were born. State birth cards and Notification of Birth Registration are not acceptable.

**A previous U S Passport book that was issued more than 15 years ago to an adult 16 years old or older. A previous U S Passport book that was issued more than 5 years ago to a child who was 15 years old or younger at issuance but who is now 16 years or older.

**An original Naturalization Certificate (if you are a naturalized citizen).

**A Certificate of Birth Abroad (if you were born to U S parents abroad).

If you have none of the citizenship evidence listed above, please call our offices for more information.

 

You will also need:

**A new passport photo. The photo must be 2” X 2”, have a white or off-white background and have been taken within the past 6 months. The Douglas County Clerk’s Office in Roseburg, travel agencies and professional photographers can take passport photos for you. Photo’s taken at the Douglas County Clerk’s Office are $15.00 (check, cash, or credit/debit card) and are taken between the hours of 9:00 am and 4:00 pm. The Douglas County Annex in Reedsport does not take passport photos.

**A valid Driver’s License or other government issued valid ID (like a Military ID). Note: Oregon ID cards are not acceptable by themselves. Please bring an additional piece of valid photo ID or bring a witness that has known you for 5 years or more. The witness must have acceptable valid photo ID. Both parent’s valid photo ID will be used for children 15 years old and younger. One parent’s valid photo ID can be used for children 16 and 17 years old if the child does not have acceptable valid photo ID. If the child has any kind of photo ID (school student body card, Oregon ID card etc) please bring it as well.

**A completed DS-11 application that is available at our offices or online at the U S Passport Services website, travel.state.gov.

Special Circumstances for Children:

**Both parents must accompany a child 15 years old or younger when they are applying for a passport. If there are special circumstances that prevent both parents from appearing, please call our offices or refer to information provided on the U S Passport Services website (travel.state.gov). If the child has an expired passport, please make sure to bring the expired passport and a certified copy of the child’s birth certificate. Both parents will be asked to show valid photo ID and will sign for the child.

**Children 16 and 17 years old must be accompanied by at least one parent when applying. That parent will be asked to show valid photo ID and co-sign the application with the child. Children 16 and 17 years old will be issued a 10 year adult Passport. They can use an expired child’s Passport as proof of citizenship even though it was issued to them when they were 15 years old or younger.

Credit/debit cards can only be accepted for the Clerk’s Execution fee. Passport Services requires a separate check or money order for their fee.

U S Passport book fee: A check or money order made out to U S Dept of State for:

Adults (16 years old or older)…………………… $110.00
Child (15 years old or younger)…………………… $80.00

 

U S Passport card fee: A check or money order made out to U S Dept of State for:

Adults (16 years old or older)…………………….. $30.00
Child (15 years old or younger)…………………... $15.00

Note: You may apply for both a Passport book and card on the same application. Only one $25.00 execution fee will apply.

 

Expediting:

Normal processing time is 4 to 6 weeks.

If you need your passport within 2 to 3 weeks, an additional fee of $60.00 for each passport will be required. An Overnight Delivery Return Fee may also be added to expedite return mail from U S Passport Services. The Overnight Delivery Return Fee is currently $12.85. Even with expedite fees we cannot guarantee your passport will arrive in time.

If you need your passport in less than 2 weeks, you must apply in person at the nearest Passport Agency. Our area is covered by the Seattle Passport Agency. Please call for an appointment with them at 1-877-487-2778. You will need to have all of the same items that would be required if you came into our offices to apply, plus proof of your travel (ticket, itinerary etc) In addition to the regular passport fee, the $60.00 expedite fee will be charged at the Passport Agency. Renewals:

Normal Processing time is 4 to 6 weeks. If you need it faster, please see “Expediting” above.

You must renew by mail if your current U S Passport book was issued to you less than 15 years ago and you were at least 16 years old or older at the time of issuance. Children’s passports (issued at 15 years or under) cannot be renewed by mail, however the expired passport can be used as proof of citizenship for an application for a new adult passport.

 

To Renew you must send:

Your most recent renewable passport.

A completed DS-82 Application for Passport by Mail form. This form is available at our offices or online at travel.state.gov.

A new Passport photo.

A check or money order for $110.00 made out to the U S Dept of State. You will mail all of these items to the address shown on the inside of the DS-82 Application for Passport by Mail form..

 

If your passport book or card is lost or stolen, please call our offices as soon as possible or call 1-877-487-2778.
